The Knowledge Is Already in the Room

Jonathan Batchelor, CEO of What Caused This, explains why public-sector organisations need to turn individual problems into shared understanding, better decisions and lasting organisational learning.

Public-sector organisations are not short of information.

Incidents, complaints, audits, service failures, performance measures and improvement plans all generate records. Most organisations already have systems to capture that information, manage workflows and track actions.

But recording what happened is not the same as understanding why it happened.

The knowledge needed to understand a problem rarely sits in one place. It is spread across frontline teams, specialists, managers, contractors, partner organisations and the people who use public services. Each may hold a different part of the picture.

That matters particularly in public services, where problems often cross teams, organisations and professional boundaries. Decisions, processes, technology, working conditions, communication and human behaviour can all interact. The people closest to the work often understand practical realities that cannot be seen in a dashboard or incident record.

The real opportunity is to bring those perspectives together.

For more significant problems, that may mean a structured investigation. For others, a simpler analysis may be enough. Not every problem needs the same level of scrutiny, but every problem can contribute to what the organisation knows.

Technology has an important role to play here, but it should support human understanding rather than attempt to replace it. It should help people organise what they know, explore how different factors connect, challenge assumptions and build a shared understanding of what really happened.

The bigger challenge comes next.

Too often, valuable learning remains within a report, a meeting or the team directly involved. Actions may be completed, but the organisation can still struggle to answer the more important questions.

Are the same causes appearing elsewhere? Has another team already seen something similar? Did the changes made work? What can this problem teach the wider organisation?

This is where individual analysis becomes organisational learning.

What Caused This is designed to work alongside the systems organisations already use. It provides a dedicated place to understand problems, connect causes to actions and outcomes, identify recurring patterns and retain learning beyond the people directly involved.

It is not about collecting more information for the sake of it.

It is about making better use of the knowledge that already exists across the organisation.

Not a warehouse for incidents. A place where people turn evidence and experience into understanding, and understanding into lasting improvement.

Because solving the immediate problem matters. Making sure the organisation learns from it matters even more.
